Request for access to your personal information

If you would like to see information you think we hold about you, you need to complete a Subject Access Request Form MS Word 20Kb and read the associated details MS Word 45Kb, available to download from our web site (MS Word 2000) or from the Service Data Protection Officer at HFRS. You may appoint someone else to apply on your behalf such as a parent/ guardian or solicitor. Subject access request forms must be submitted in paper format to the HFRS Data Protection Officer, as signatures and evidence of identification are required (e.g. driving licence, birth certificate, recent utility bill).

In order to see your information, you must provide sufficient details that will enable us to search our records and locate your information. On receipt of the completed Subject Access form and payment you will receive a letter of acknowledgement and an indication of when you will receive the reply, which will be within the statutory 40 day period.

A cheque or postal order for £10 (current statutory fee) made payable to Hampshire Fire and Rescue Service is to help cover our administration costs.
